The Lapel Pin Order Form must be completed in order for Smart Serve Ontario to replace your Lapel Pin.
This form contains all the information required by Smart Serve Ontario to replace your Lapel Pins.
Lapel Pins cannot be sold to anyone who has not successfully completed the Smart Serve Training Program.
