Replacement Card - Individuals requiring their
certificate replaced, must put their request in writing providing the information contained in this order form.

Cover sheet - This form is essential to avoid delays in
processing your test, you must send a Completed Cover Sheet with each batch of completed test(s)

NOTE: When mailing tests to us please take a minute to ensure that all handwriting is legible and that every participant has signed
their tests. Tests without a signature will be mailed back unprocessed.
